As a participant in our programs, you have the right to receive care without discrimination. We welcome all individuals regardless of race, age, ability, background, or belief. You deserve respectful treatment, full transparency from our professionals, and clear information about your care, options, and outcomes.
You have the right to make informed decisions, to refuse treatment within legal limits, and to expect confidentiality in your records and relationships. If you ever feel your care falls short, you may submit a grievance or appeal.
We also ask that you engage with mutual respect—toward staff, fellow participants, and the space we share. We encourage you to ask questions, understand your care, and participate actively in your wellness journey. If you choose to decline treatment, we trust you’ll do so with awareness of the risks involved.
When participating in physical activities, you acknowledge that some risk is involved. You are responsible for knowing your limits and supervising any children who accompany you. Children and teens must remain in designated areas and be monitored at all times. By joining our programs, you waive liability for injuries and agree to the terms outlined in our waiver.
If you consent to media use, you allow Family Service Association to photograph or quote you for educational and promotional purposes. This may include public service announcements, advertising, or web content. You waive privacy rights related to this use and release us from liability connected to publication.
For USDA-supported programs like TEFAP, we do not require participation in religious activities and will make efforts to refer you elsewhere if needed. You have the right to report any violations of these protections to the USDA or State agency. We are an Equal Opportunity Provider.
